Parents for your convenience, all of the forms that you need to enroll your child into our facility are here at your fingertips. From your smart phone or computer open the documents in Google docs or another editing app.  Fill out all portions and email them back to mfgentrygmlc4k@gmail.com 

Each family must also completely fill out all of the forms shown own this page which includes the food program application before being accepted into the program.

After all documents have been emailed , Parent orientation will be scheduled via Zoom.
